Sepsis Patients Monitoring with PraediAlert
New tool for pharmacists! We’re excited to announce the launch of the Sepsis Patients Monitoring Module, a new tool designed specifically to help pharmacists closely monitor patients who meet Systemic Inflammatory Response Syndrome (SIRS) criteria. This module ensures timely reassessment and appropriate care coordination to meet CMS sepsis standards.
Who is Included in the Module?
The module automatically includes patients who meet at least two of the following four SIRS criteria within a 6-hour window:
- Temperature > 100.9 or < 96.8
- Heart Rate > 90
- Respiration > 20
- WBC > 12 or < 4, or Bands % > 10
To allow for ongoing re-assessment, a patient will be included again (creating a new line entry) the first time on each calendar day that they meet criteria.

Key Features for Pharmacists
The module aggregates critical patient information, reducing the need to navigate multiple systems.
1. Patient Assessment and Documentation
Use the dropdown selectors right in the module to document your assessment of the patient’s status to Pending, Yes, or No for each:
- Sepsis
- Severe Sepsis
- Septic Shock
You can also use the Status dropdown column for internal tracking (New, Follow-Up, Completed).
2. Direct Intervention and Review
- Intervene Button: Click the red Intervene button to quickly document an intervention related to the module
- Chart Links: Click Patient Chart-Daily Snapshot ID to view the daily snapshot, the paper icon to enter a Note, or the envelope icon to enter a Message for the patient. Clicking the Patient SSN brings up the full PraediAlert patient chart.
3. Critical Monitoring Data at a Glance
The module provides immediate visibility into key diagnostic and treatment information:
